What is myofunctional therapy?
Myofunctional therapy (sometimes called orofacial myology) is the evaluation and treatment of the muscles of the face, mouth, and throat. Specifically, it focuses on how those muscles function for:
- Nasal breathing — the default mode we are designed to use.
- Lip seal at rest — lips closed, gently, without strain.
- Tongue posture at rest — the whole tongue resting against the palate, not the floor of the mouth.
- Swallow pattern — a tongue-up, no-lip-lurching swallow.
- Speech articulation — clear sounds produced by coordinated muscle patterns.
- Facial and dental development in children — the way muscles shape growth.
When one of these patterns breaks down — from chronic mouth breathing, prolonged thumb-sucking, tongue-tie, or habit — the others often follow. Myofunctional therapy retrains the system to work the way it is designed to.
Treatment looks like targeted exercises: tongue lifts, swallow practice, lip-strength work, breathing drills. Sessions are weekly or bi-weekly; daily home practice is essential.
Who myofunctional therapy helps
We see children (often starting around age 4–5) and adults of all ages. The most common reasons Louisville families are referred to 502 MYO:

The first visit is a 60-minute, in-person evaluation. We look at oral posture at rest, swallow pattern, tongue mobility, breathing mode, lip seal, and related muscle function. We discuss findings at the end of the visit and leave you with a written plan.
Therapy programs typically run 4–9 months:
- Weekly for 4–8 weeks — establish the new patterns, build strength, troubleshoot barriers.
- Every 2–4 weeks for 3–6 months — reinforce and generalize the patterns to daily life.
- Tapering — check-ins at increasing intervals as the new patterns become habit.
Home practice is non-negotiable. Most patients do 10–15 minutes daily. Programs where home practice is skipped do not work.
How to choose a myofunctional therapist in Louisville
Myofunctional therapy is not a licensed clinical profession in Kentucky — anyone can hang a shingle. That makes vetting important. Look for:
- Formal training through a recognized program (IAOM, Neo-Health Services, or equivalent).
- A full 60-minute evaluation — not a "free screening" or a 15-minute consult.
- Coordination with your other providers (dentist, orthodontist, ENT, SLP) and willingness to share progress notes.

We do not bill insurance directly. We can provide a superbill for out-of-network reimbursement — submit it to your insurance and they may pay you back. HSA/FSA cards are accepted.

When myofunctional therapy isn't the right next step
We will tell you. Some situations need medical, surgical, or other specialist care first:
- Enlarged tonsils/adenoids causing airway obstruction — ENT evaluation first.
- Undiagnosed sleep apnea — sleep medicine workup before myofunctional therapy.
- Active TMJ pain with joint damage — dental/TMJ specialist evaluation first.
- Tongue-tie release needed before therapy can take — coordination with the release provider.
- Behavioral or developmental issues better served by another specialty.
If we don't think therapy is right for you, we'll say so and refer you to the right provider. We coordinate care with your existing team as needed.
